Hand with Dupuytren's Disease

Dupuytren’s Disease

Dupuytren’s disease is a progressive condition that affects the connective tissue within the hand. It is characterised by the thickening of the fascial bands in the palm and fingers, which over time can lead to formation of nodules and cords. This causes discomfort, restricts range of motion of the hand and has an adverse impact on daily activities.

  • Surgical Dupuytren’s correction aims to release or remove the abnormally thickened tissue:

    • It creates a ‘firebreak’ in the disease progression

    • It helps straighten the fingers, improving hand function

    • It can help reduce pain and discomfort caused by Dupuytren’s nodules and cords

    • All of the above help to improve your ability to complete everyday tasks

There are several types of surgery for Dupuytren’s contracture release and the technique used depends on the anatomy and severity of your disease:

    • Needle fasciotomy is a minimally invasive technique, where a needle is used to release the cord providing (often temporary) relief from the contracture

    • Fasciotomy is a more involved procedure requiring small incisions to release the contracture

    • Fasciectomy and dermofasciectomy are more extensive procedures involving removal of the affected tissue including the Dupuytren’s cords and in some cases, the skin. In these cases wounds are closed in a zig-zag fashion and occasionally require a small skin graft.

Depending on the extent of the operation, the surgery will take between 30 minutes - 2 hours, and may be performed under regional or general anaesthetic. The recovery process is different for each patient, but you will likely be able to resume work and regular activities within a few weeks. The results are visible immediately with digits straightened and cords absent, however the long term outcome is very much individual and depends on the severity of the disease. Functional recovery can take a few weeks and usually needs support by hand therapists.

It is important to note that the primary goal of surgery is to improve hand function, and there is currently no cure for Dupuytren’s disease. The disease tends to carry on progressing and may require further surgical corrections in the future.

Hand with carpal tunnel syndrome

Nerve Compression Syndrome

Nerve compression syndromes encompass a number of conditions (such as carpal tunnel and Guyon’s canal in the wrist, lacertus / pronator syndrome in the forearm or a cubital tunnel at the elbow), where nerves become compressed or pinched, leading to pain, numbness, tingling or weakness of the limb. These symptoms can significantly impact daily life, activities and functionality.

  • The surgery is performed either under local or regional anaesthesia and involves release of tight structures such as carpal ligament or fascial layers to free up the nerve.

The duration of surgery varies depending on what exactly is being done and can range from 15 minutes to 1.5 hours. It is done as a day case procedure, meaning you are able to return home the same day. You will have a bulky bandage postoperatively, which can be reduced after a few days. In some cases the help of a hand therapist may be required to aid your rehabilitation. The period of recovery is individual, with some patients returning to their normal activities within a couple of weeks, while some will take 6 weeks or more.

  • “Professional and friendly service and always available to chat with any worries I had.”

Hand with a ganglion cyst

Ganglion / Lesion Treatment

Ganglion cysts are benign lumps appearing on the hand or the wrist, sometimes causing discomfort and pain, especially if caught on various objects; when located on the fingertip it can cause deformity of the nail plate.

  • Surgical excision of a ganglion helps improve these symptoms and restore your confidence in the appearance of your hands.

The procedure is performed under either local or regional anaesthetic on an outpatient basis, meaning you can return home the same day. During surgery the cyst is removed, and every effort is made to reduce the likelihood of the cyst returning, however this cannot be guaranteed.

The operation will take around 20 minutes - 1 hour, and you may experience some pain or bruising in the area following surgery. Depending on your job, you may be able to resume work immediately, however if your work involves manual labour it may be necessary to take some time off in the initial stages of recovery.

  • “When I had worries all my questions were answered it was relatively easy to get hold of anyone I wanted to speak to. I’m really pleased with my experience and the result!”

Hand with trigger finger

Trigger Finger Release

Trigger finger (also known as stenosing tenosynovitis) is a condition where one of the digits gets stuck in a bent position and then straightens with a snap or a click - resembling pulling and releasing a trigger. In more severe cases it may get stuck and not straighten. This condition occurs due to inflammation and narrowing within the sheath that surrounds the tendon in the affected finger.

  • The release of a trigger finger is a surgical procedure performed to relieve the triggering (locking):

    • It can eliminate the sensation of locking, clicking or catching of the digit

    • It can restore smooth finger motion

    • It can relieve pain and discomfort associated with the condition

It is done as a day case procedure under local anaesthesia. The recovery is typically quick with patients being able to return to office based jobs within a couple of days, while it may take a couple of weeks to get back to manual work.

  • “I would like to thank all the staff that took very good care of me during my procedure. Felt at ease and am extremely happy with my results.”
